Community Access Services
Providing easy access to services can prevent a mental health problem from escalating and spiraling into a crisis that can lead to homelessness or hospitalization. Through our Community Access services we help people navigate through the system to understand what resources are available.
It begins with an initial phone screening, or use the simple online inquiry form, to identify the individual’s needs. Then, we can complete a Diagnostic Assessment, enabling us to refer to ongoing services, including Guild’s community-based Assertive Community Treatment (ACT), Integrated Case Management, and Behavioral Health Home Services.
