Coming Home Pilot Project: a New Partnership

In partnership with Hearth Connection, HealthEast St. Joseph’s Hospital, and Catholic Charities, Guild has launched the Coming Home Pilot Project.

The goal of Coming Home is to improve quality of life for individuals experiencing homelessness and to reduce unnecessary emergency or inpatient visits. Utilizing the expertise and resources of each partner, the pilot project will work to secure supportive housing for people being discharged from the mental health unit at St. Joseph’s Hospital.

Staff at St. Joseph’s Hospital will identify individuals eligible for the project, and Guild staff will make initial contact with patients prior to their discharge.

In most cases, individuals leaving the hospital will move to temporary housing at Higher Ground, Catholic Charities new housing program in St. Paul. Guild staff will work with the individual and his or her support system to establish ongoing housing and the services needed to achieve success in maintaining and keeping their home.

Hearth Connection will complete evaluation of the program as well as ensuring housing subsidies.

This new pilot project is similar to Guild’s Delancey Street and Hospital to Home (H2H) programs.
