REALIZATION of Delancey Apartments , which provides permanent, supportive housing for people experiencing chronic homelessness who have had difficulty maintaining their housing.  Resulting from collaboration between us and Project for Pride in Living (PPL), Delancey Apartments features easy access to nursing and employment support.

PARTNERSHIP with other providers “Hospital to Home,” a project aimed at identifying efficient, cost effective ways for individuals with mental illness experiencing homelessness and chronic health conditions to get the care they need without relying on high cost emergency room care.

IMPLEMENTATION of Credible Behavioral Healthcare software, which integrates scheduling, service documentation, and billing. The new technology helps staff focus on what they do best – provide quality service to clients, while meeting record keeping responsibilities in the most efficient way.
